Portal privacy information

The portal stores your contact details, appointments, messages, documents and records entered by your clinical team. Assigned clinicians can access clinical information; assigned coordinators manage scheduling and contact details. Administrators manage enrollment and access without automatic clinical access.

Microsoft provides sign-in. Hosting is configured separately for the clinic. International access requires clinic review.

You can download your shared record and ask for corrections through Record requests. Signing out does not delete clinical records. Retention and lawful deletion are determined by the clinic's records policy.

Messages are not an emergency service. Contact the clinic directly for urgent care and local emergency services for emergencies.

This operational notice requires clinic approval and clinic contact details before patient launch. It does not replace the clinic's legal notice.
