Create patient account

Patient access is being prepared. Account requests and patient intake are not open yet.

After clinic approval, enter your contact details and health history in the secure portal. Choose SMS, automated voice calls and email separately; none is selected automatically.

1. Sign in with your own verified identity.
2. Review the privacy notice and request your account.
3. The clinic checks the request and matches the correct chart.
4. After approval, complete Personal, Medical, Location and Review intake sections.

Existing clinic records are not matched by name or email automatically. Never share another person's account or password.

For access help, contact the clinic through its existing contact method. Do not email medical records or identity documents.
